If an individual meets one or more of the following criteria, a Skilled Nursing Home would be appropriate:</p>
<ul>
<li>
Can no longer care for themselves due to physical, emotional or mental issues;</li>
<li>
Needs help with their personal needs such as eating, toileting, bathing, ambulation, or taking medications (custodial care);</li>
<li>
Cannot live alone and requires more care than their caregiver can provide;</li>
<li>
Has a tendency to wander away if unsupervised;</li>
<li>
Has extreme medical needs requiring daily attention or monitoring by an RN supervised by an MD;</li>
<li>
Is going to be discharged from the hospital and requires temporary Skilled Nursing care or rehabilitation before returning home or to a residential facility;</li>
<li>
A physician has recommended Nursing Home care.</li>
</ul>
<p>
If a senior requires extensive assistance and has very limited income, perhaps only Social Security, he may have few alternatives but to move to a skilled nursing facility for long term care covered by Medi-Cal (see <a href="http://www.medi-cal.ca.gov/" onclick="_gaq.push(['_trackPageview', '/vpv/outlink/http://www.medi-cal.ca.gov/']); return true;" target="_blank">http://www.medi-cal.ca.gov/</a>).</p>
